Complete this form AFTER you have scheduled your recital with Mr. Boyer (Office 497C, Glidden Hall). This form (including payment/receipt) should be completed no fewer than 14 days (two weeks) before your scheduled recital date - immediately following your Recital Hearing.
This form will be used to collect information for faculty and guest recitals as well as ensembles.
This form should be completed no fewer than 7 days (one week) before the scheduled recital date, to give us ample time to format and print the program.
